One of the easiest methods to get bokeh is to place the subject and the background appropriately with maximum zoom on the lens.

For example, Consider the following diagram:

D1: Distance between the Camera and the Subject
D2: Distance between the Subject and the Background.
If D2 > D1
(or)
D2/D1 ratio is high
Then the boken effect will be maximum.
Following is the image shot on Nikon D7000 with maximum zoom of 200mm.
The subject was around 2 meters away from Camera.
The background was over 700 meters away from the subject.
